How to increase concentration:

Have a study environment
Organize your study materials 
Take breaks
Set specific times/use timer http://www.online-stopwatch.com/countdown-timer/
Make a timetable
Close access to internet and mobiles(Easier said they do right?)try an App
Have enough rest
Have some snacks/sweets in hand, they are very important
Listen to music if that helps
Allow fresh air
"Write down list of things you wanted to do, when revising. Do them in the breaks(Like me now hehe)"
Finally when you start to lose concentration try to think less have 5 more mins or last 3 questions, before you can do what you wanted to do.
